What is IPC?

 

The Langs Interprofessional Primary Care (IPC) Team is a multidisciplinary group of healthcare professionals that works together to provide comprehensive care to the community of Cambridge. The team provides outreach and health care to those who are unhoused, precariously housed or face other barriers to care. The IPC Team provides a coordinated approach, working with other community partners at various community sites as listed below.

 

Eligibility

 

  • 18+
  • Does not have access to a health care provider
  • Living unhoused for Clinics at 150 Main Street, 1145 Concession Road
  • Living housed, unhoused, or at risk of homelessness at 562 Concession Road
  • Living in the shelter system at 26 Simcoe Street or other shelter spaces

 

Our Services

 

  • Primary medical care, including diagnosis, treatment, and follow-up for various health conditions.
  • Support for mental health issues, counselling, and referrals to community resources.
  • Personalized support, appointment accompaniment and connection to community resources.
