CHCs (Community Health Centres) are the only primary health care model in Ontario that, under one roof, combines primary health care services with a wide range of other health promotion and community development services.

CHCs build inter-professional teams working in collaborative practice. The Langs inter-professional team includes physicians, nurses, nurse practitioners, dietitians, social workers, health promoters, community development workers for early years, youth, adults and seniors, fundraisers and administrative staff.
