Regional Coordination Centre (RCC)

The Regional Coordination Centre is hosted by Langs and is funded by the Waterloo Wellington Local Health Integration Network (LHIN). The RCC’s role is to coordinate services and to support individuals and health care providers in accessing services.

Central Intake provides one location to receive, triage and direct referrals to the appropriate centre for care throughout the region. It ensures people are accessing the right care at the right place at the right time.

Central Intake for Diabetes includes referrals to diabetes education programs, diabetes specialists, ophthalmologists, and nephrologists.

Central Intake for Medical Specialists currently includes Orthopedic Central Intake with plans to expand to other services. The team are working with the eHealth Centre of Excellence to adopt an eReferral solution.

The RCC’s role is also to support health care providers in enhancing their diabetes knowledge, skill and care. This is accomplished through both the diabetes website and the mentoring program where an experienced certified diabetes educator visits the clinic or office, and participates or observes the education session and supports the clinician during a patient visit. This service is available to health care providers throughout the region.
